Experiencing Ache Knees and Back? You may have Osteoarthritis

Are you noticing as you mature you are experiencing stiffness or
pain in your knees, ankles, back, and elbows? If you are 35 and
above symptoms like these may be the early signs of
Osteoarthritis.

What is Osteoarthritis?

According to the US government’s National Institute of Arthritis
and Musculoskeletal and Skin Diseases “Osteoarthritis is the
most common type of arthritis, especially among older people.
Sometimes it is called degenerative joint disease or
osteoarthrosis.”

Moreover Osteoarthritis is a joint disease that mostly affects
the cartilage. Cartilage is the slippery tissue that covers the
ends of bones in a joint.

Unlike other forms of arthritis which may affect internal organs
Osteoarthritis primarily causes problems between the cartilage
and joints.

Healthy cartilage allows bones to glide over one another. It
also absorbs energy from the shock of physical movement. In
osteoarthritis, the surface layer of cartilage breaks down and
wears away.

In other words as we mature you can start to experience
Osteoarthritis because the cartilage between you knees, back,
and other joints may begin to erode or wear away.

This allows bones under the cartilage to rub together, causing
pain, swelling, and loss of motion of the joint. Over time, the
joint may lose its normal shape. Also bone spurs, small growths
called entophytes, may grow on the edges of the joint. Bits of
bone or cartilage can break off and float inside the joint
space. This causes more pain and damage.

According to government studies Osteoarthritis affects about 20
million Americans. Over 50 % of Americans over 65 have
osteoarthritis in a least one joint. It is more common for men
than women to have osteoarthritis before 45. However after 45 it
is more common for women to experience the symptoms of this
condition. By 2030 70 million Americans will be over the age of
65 and may be at risk for osteoarthritis.

While younger people may get osteoarthritis from sports related
knee and joint injuries osteoarthritis is primarily a condition
more mature people experience. If you experience symptoms like
the ones described above talk to you doctor to see if you have
osteoarthritis or some other condition.

The symptoms of osteoarthritis seem to appear in the knee, lower
back, ends of the fingers, back of the neck, thumbs and hips.

Some of the treatments the doctor may prescribe are:

1. – Exercise is inexpensive and may help with a number of
health problems including certain forms of osteoarthritis. As
you may know exercise can help improve your attitude,
disposition, mood and overall outlook, decrease pain, increase
flexibility, improve the heart and blood flow, maintain weight,
and promote general physical fitness.

Reports have shown if done correctly, it has few negative side
effects. The amount and form of exercise will depend on which
joints are involved, how stable the joints are, and whether a
joint replacement has already been done.

The proper exercise may help you recapture some range of
movement in stiff joints.

2. – Weight control: Osteoarthritis patients who are overweight
or obese need to lose weight. Weight loss can reduce stress on
weight-bearing joints and limit further injury. A dietitian can
help patients develop healthy eating habits. A healthy diet and
regular exercise help reduce weight.

3. Medicines: Doctors prescribe medicines to eliminate or reduce
pain and to improve functioning. Doctors consider a number of
factors when choosing medicines for their patients with
osteoarthritis. Two important factors are the intensity of the
pain and the potential side effects of the medicine. Patients
must use medicines carefully and tell their doctors about any
changes that occur.

4 – Surgery : As a last resort a surgery to replace a joint may
be in order from your doctor.

For many people, surgery helps relieve the pain and disability
of osteoarthritis. Surgery may be performed to remove loose
pieces of bone and cartilage from the joint if they are causing
mechanical symptoms of buckling or locking Resurface (smooth
out) bones Reposition bones Replace joints. Surgeons may replace
affected joints with artificial joints called prostheses. These
joints can be made from metal alloys, high-density plastic, and
ceramic material.

Arthritis is a term used to describe a number of diseases that cause inflammation of joints and results in the progressive destruction of joints. Arthritis is actually a collection of many different joint diseases that affect the body and the foot in many different ways. In this article, let’s talk a bit about arthritis and how it affects the foot. We normally discuss treatment alternative for conditions, but due to the scope of this topic, we will not discuss treatment of each of the individual forms of arthritis.

Osteoarthritis

Osteoarthritis is by far and away the most common form of arthritis. Osteoarthritis is known as the wear and tear type of joint disease and will effect most of us at one point in our lives. In our discussion on osteoarthritis, please refer to the nomenclature and anatomy sections below for definitions of the terms that my be unfamiliar to you.
When I think of osteoarthritis (OA), I think of my grandmother’s hands. Those hand have done a lot of work in their time and have a bit of arthritis to show for it. The fingers, crooked and bumpy show many of the changes brought on by osteoarthritis. The bumps on the fingers are called Heberden’s Nodes. Heberden’s Nodes are a very obvious example of how osteoarthritis occurs. If you’ve ever known anyone whose experienced the symptoms of OA, they’ll tell you that a joint will flare for a period of several weeks. It may be mildly painful or even painful to the degree that the joint is unusable. And then suddenly, just as passively as it came, it leaves. Occasional, brief morning stiffness was not unusual for grandma. The pain that had been present in her hands was actually due to a series of small micro-fractures that occurred in the subchondral bone (just beneath the cartilage). It seems that in cases of osteoarthritis, the bone just below the cartilage becomes very fragile. When loads are applied to this fragile bone, it breaks down. The more the joint is used, the more the bone seems to break down.
Over time, as the bone undergoes a series of flare ups, the ability of the bone to support the cartilage becomes unstable. The subchondral bone creates an unstable supporting surface for the cartilage. When the joint tries to complete its’ normal range of motion, the cartilage is irregular and progressively erodes. This moves us into the second phase of OA pain, and that’s when we have joint range of motion that is bone on bone. Cartilage has no nerve endings to sense pain, but bone on the other hand, has plenty. As the joint moves and the bone is eroded, significant pain can be experienced.
This erosive process seen in OA seems to occur at different rates in everyone. In fact, every joint seems to undergo the changes of OA at a different rate. The rate of change does seem to be effected by the history of damage to the joint. In fact the classifications used to describe OA include Primary (idiopathic or no known cause) and Secondary (some known cause for the disease such as trauma). Past injuries seem to accelerate the rate of OA in most joints. We all know someone with that old football injury, right?
The cause of OA is unknown. Many authors have speculated that the cause may be mechanical as described above, but others have described changes such as biologic, biochemical or enzymatic; or even a combination of each of these.
Changes that occur in the foot with OA are numerous. The most common is midfoot pain that is diffuse. The midfoot is a jigsaw puzzle of pieces that are difficult to manage when effected by a systemic disease of this nature. Other finding include atrophy (loss) of the plantar fat pad of the forefoot and heel. The big toe joint is often effected by the changes of OA with bunions and hallux limitus as common complaints.

Rheumatoid Arthritis

Rheumatoid arthritis (RA) is a very complex and debilitating disease that affects approximately 1% of the general population. Women are 2-3 times more prone to develop RA. RA is a process where the body’s immune system attacks the cell lining the joint capsule (synovial cells). The end result is thickening and fibrosis of the joint with destruction of the cartilage.
Symptoms of RA include an abrupt onset of inflammation. The swelling associated with RA may be isolated to one joint or effect several. Symmetrical involvement is typical with both hands or both feet being affected. In comparison to OA, morning stiffness seen in RA may last more than 30 minutes.
The finding of RA in the foot often progresses to subluxation of the joints, particularly the metatarsal phalangeal joints. Atrophy of the plantar fat pad is common.

Psoriatic Arthritis

Psoriatic arthritis (PA) is an interesting form of sero-negative arthritis. PA is associated with psoriasis in many cases, but it is not unusual to find the symptoms of PA in a patient with no previous history of psoriasis of the skin or nail.
The symptoms of PA in the foot include focal swelling of the interphalangeal joint (the toes). The toe will appear to have swollen and become painful with a limited history of injury or pain. X-rays often show slow changes that exhibit erosion called a pencil and cup appearance. This finding is common on the metatarsal phalangeal joint.

Reiter’s Syndrome

Reiter’s Syndrome (RS) is an unusual form of arthritis that is usually found in men.. RS is often described as a sexually transmitted disease due to the history of the disease occurring following sexual intercourse which resulted in a chlamydial infection. It is suggested to treat those with RS, and their sexual partners, for C. trachomatis infections.
The symptoms of RS include burning upon urination (urethritis), dry eyes and joint pain. Joint pain in the back and feet are common. These symptoms occur 7-14 days following sexual intercourse and subside over a period of several months to years. Recurrence of pain is not unusual. About the Author

Yoga is an ancient Indian science and way of life, which influences the functioning of the brain and the rest of the body. Several studies have shown that the practice of yoga has a definite role in the promotion of positive health, including mental health, characterized by improved cardio-respiratory efficiency, autonomic responses to stressors, sleep, muscular endurance, and `higher brain functions. With an increase in the incidence of stress-related ailments, related to the rapid pace of life today, yoga has been evaluated as a treatment for such disorders in several controlled trials. The disorders, which were most likely to respond to yoga, with reduced symptoms and need for medication, were bronchial asthma, non-insulin dependent diabetes mellitus, anxiety neurosis, and rheumatoid arthritis. THE CONCEPT OF HEALTH According to the World Health Organization (WHO) the state of health is defined as a state of complete physical, mental, social and spiritual well being and not merely an absence of disease or infirmity. It is clear from this definition that health and ill-health are not two discrete entities as commonly understood but health should be conceived as a continuous function indicating the state of well being.The ancient Indian science, Yoga, has its origin in the Sankhya philosophy of Indian culture, which is about 8000 years old (Nagarathna, 2001). Yoga includes a wide range of techniques (e.g., physical postures, regulated breathing, cleansing techniques, meditation, philosophical principles, and devotional sessions, surrendering to the Supreme). These techniques bring about a calm and balanced state of mind, and are expected to help the spiritual evolution of the individual. However, yoga has more pragmatic applications in medicine. In order to understand these, it is important to know the concepts of health and disease in Yoga texts. According to yoga, man is in perfect health and homeostasis at his subtle levels of existence. All diseases are classified as (i) stress-related (adhija) and (ii) not stress related, e.g., injuries (anadhija) (Vasudeva, 1937). Yoga has been considered especially useful in the management of stress related disorders by getting mastery over the excessive speed of the mind. The technique to reducing the rate of flow of thoughts with deep internal awareness is yoga. This review will describe the therapeutic applications of yoga in the management of arthritis. Physical activity is an essential part of the effective treatment of osteoarthritis (OA) and rheumatoid arthritis (RA), according to treatment guidelines published by the American College of Rheumatology (Hochberg, 1995; Newsome, 2002). In persons with arthritis, exercise is safe and does not exacerbate pain or worsen disease (Ettinger,1997; Minor,1999; O’Grady,2000;Bearne,2002). In fact, exercise may play a key role in promoting joint health (Forrest,1994) , since those who do not exercise often suffer more joint discomfort than those who do (Nordemar,1981). However, regular physical activity is especially important for people with arthritis, who often have decreased muscle strength, physical energy, and endurance (Lyngberg,1988). The psychological benefits of exercise such as stress reduction, fewer depressive symptoms, improved coping and well-being and enhanced immune functioning (Taylor,1985;Scully,1998;Fox,1999 Paluska,2000) also contribute to greater overall health.Scientific studies on yoga Yoga has been used in the management of a wide range of diverse ailments. While there is a great deal of anecdotal evidence of the benefits of yoga, to date only a handful of scientific studies have been conducted on persons with OA and RA (though several more are currently underway). The study conducted in England demonstrated that hand grip strength significantly improved following yoga in rheumatoid arthritis patients (Haslock,1994). It was subsequently proven that yoga resulted in similar benefits in rheumatoid arthritis patients in an Indian population (Dash, 2001). Studies in osteoarthritis of the hands and carpal tunnel syndrome show greater improvement in pain during activity, tenderness and finger range of motion (Garfinkel, 1994). These above studies have shown promising results with some improvement in joint health, physical functioning, and mental/emotional well-being. Perhaps most importantly, yoga has an important positive effect on quality of life. People with arthritis may also enjoy yoga more than traditional forms of exercise, and exercise enjoyment is an important predictor of adherence(Ryan,1997;Trost,2003).This is particularly important considering that, on average, 50% of sedentary individuals will drop out of exercise within 6 months (Dishman,1990).In summary, yoga can be a meaningful and enjoyable alternative to traditional forms of exercise such as aerobics or aquatic exercise with important health benefits. Yoga can play an important role in reducing stress and frustration that results from pain and disability, and increasing positive feelings and wellbeing. Drug treatments for OA and RA have improved markedly in the last few years. Despite this, arthritis cannot be cured, and even the best medications and medical care can only help a little. There is a great need for additional activities patients can do to reduce pain, disability, and take control of the overall impact arthritis may have on their lives. Thus, the evidence suggests that, when combined with a program of good medical care, yoga may provide important additional physical and psychological health benefits for arthritis patients. Finally, it has to be emphasized that while yoga has important therapeutic benefits, the practice of yoga is very important in the promotion of positive health and human potential in body, mind, and spirit (Scott, 1999).References:1.Nagarathna R.
